Providers and managers of:

  • Independent hospitals
  • Independent clinics
  • Independent medical agencies

How to register

Read the guidance for new providers and complete the application form, both of which can be downloaded below; please make sure that you enclose all required documents.

Email your completed application form and associated documents to HIWregistration@gov.wales  or post a hard copy to Registration Team, HIW, Welsh Government, Rhydycar Business Park, Merthyr Tydfil, CF48 1UZ.

Following receipt of your application we will check that it is complete and assess the documentation and information provided. Full details of the assessment process and the information we will request can be found in the guidance.

Disclosure and Barring Service (DBS) checks

You can get a DBS application form from us by calling 0300 062 8163 or by emailing HIWregistration@gov.wales. Guidance on completing the DBS form can be found here.  

HIW must process all DBS checks for new providers and managers of independent healthcare services. The cost for applying for a DBS check is currently £44.00 (which includes postal costs).

Please note: The way we process DBS applications has changed. We must now conduct identity checks in person. Please do not send your identification documents to HIW. When you request a DBS application form, we will explain how you can present your identification documents to us and pay the application fee.

Registration fees

Most applications to register an independent healthcare provider or manager must be accompanied by a registration fee. We will tell you how much the fee is and how you can pay this when you have submitted your application.

Fees are also payable:

  • When applying to vary any condition of registration
  • When applying to remove a condition of registration
  • Annually to maintain registration

The law and national minimum standards

Independent healthcare services must be registered with us as legally required by the Care Standards Act 2000

The Registration of Social Care and Independent Health Care (Wales) Regulations 2002 set out the information and documentation that must be submitted with your application to register with us.

To grant your application to register with us we must be satisfied that you will comply with The Independent Health Care (Wales) Regulations 2011 and the National Minimum Standards for Independent Health Care Services in Wales.

Documents
