Healthcare Inspectorate Wales (HIW) no longer carries out homicide reviews. These have been replaced by the Single Unified Safeguarding Review (SUSR) process, which provides a single national approach to reviewing serious incidents in Wales.
The SUSR Process
The SUSR process brings together agencies involved in an incident to enable a coordinated and timely review. It removes the need for multiple separate reviews, helping reduce trauma for families and ensuring learning is identified sooner.
An SUSR may be carried out where criteria for one of the following reviews are met:
- Adult Practice Review
- Child Practice Review
- Domestic Homicide Review
- Mental Health Homicide Review
- Offensive Weapons Homicide Review
