This report is a review of the provision of mental health services provided to Mr K prior to March 2011.
Healthcare Inspectorates Wales have been commissioned by Welsh Ministers to undertake this external review of the provision of mental health care and treatment provided to Mr K prior to the homicide committed in 2011 to determine if there were shortcomings in the care and treatment Mr K received that may have influenced or led to the events of March 2011.
