Healthcare Inspectorates Wales have been commissioned by Welsh Ministers to undertake this external review of the provision of mental health care and treatment provided to Mr I prior to the homicide committed in 2009 to determine if there were shortcomings in the care and treatment Mr I received that may have influenced or led to the events of June 2009.
