Report of a review in the in respect of Mr H and the provision of mental health services following a Homicide committed in March 2009

Healthcare Inspectorates Wales have been commissioned by Welsh Ministers to undertake this external review of the provision of mental health care and treatment provided to Mr H prior to the homicide committed in 2009 to determine if there were shortcomings in the care and treatment Mr H received that may have influenced or led to the events of March 2009.
