Inspection of Royal Glamorgan Hospital’s Emergency Department Reveals Progress and Ongoing Challenges
Healthcare Inspectorate Wales (HIW) has published its findings following an unannounced inspection of the Emergency Department (ED) at Royal Glamorgan Hospital, managed by Cwm Taf Morgannwg University Health Board.
The inspection, carried out over three days in August, found examples of good practice and dedicated staff working hard to provide safe and compassionate care in a demanding environment. The ED was generally well managed, with timely and effective triage, and ambulance handover processes that supported prompt clinical assessment. However, systemic challenges were found, common to many EDs across Wales, mainly attributed to poor patient flow throughout the hospital. Patient discharge delays led to long waits and overcrowding, with some people experiencing prolonged waits. Some patients received care in seating areas not designed for extended stays, which compromised privacy, dignity, and comfort.
Despite the systemic pressures, several positive developments were found in the department. These included the introduction of specialist nursing roles, which provided valuable support to both patients and staff. The new paediatric area impacted positively on safety and patient experience, although inspectors highlighted the need for enhanced security measures, including access-controlled doors.
Staff consistently provided respectful care in a clean, supportive setting, communicating clearly and showing compassion even under high demand. Patients and families reported positive experiences, noting staff professionalism and dedication. Staff welcomed recent changes and recruitment efforts but some expressed concerns about burnout and limited capacity to meet rising demand. The health board should consider this feedback when planning future changes.
The department was generally well-organised and generally maintained good infection, prevention and control practices, although inspectors reminded some staff around hand hygiene, and the need to repair or replace damaged seating. Staff training compliance needed improvement, particularly for immediate and advanced life support for adults and children. Inspectors also highlighted the need for better access to food and drink for those staying longer periods.
The health board has produced a comprehensive improvement plan with detailed actions on how improvements will be made at the emergency department.

The challenges faced by the Emergency Department at Royal Glamorgan Hospital are not isolated, emergency departments across Wales are experiencing sustained operational strain, driven by patient flow challenges, high attendance volumes, and delayed discharges. HIW will continue to work closely with Cwm Taf Morgannwg University Health Board to monitor progress and support ongoing improvements.”
