Improvements noted at Ysbyty Cwm Cynon’s Dementia Care Unit

Healthcare Inspectorate Wales (HIW) has released a report following an unannounced inspection of a specialist mental health ward at Ysbyty Cwm Cynon in Rhondda Cynon Taf. Part of Cwm Taf Morgannwg University Health Board, Ward 7 is a 14-bed unit providing dementia care. The inspection took place over three consecutive days in January 2025.

We found that significant progress has been made since our last inspection in 2019, and our review of the health board in 2022. Inspectors highlighted notable improvements in care planning, audit activity, and discharge processes, showing the health board's commitment to learning and continuously improving care.

Inspectors found that care was well-coordinated and responsive, with staff showing a strong understanding of patient needs. Leadership was committed, and there was a positive, open culture where staff felt confident raising concerns. We found many positive examples of quality improvements and learning activities taking place on the ward, including newsletter bulletins, staff wellbeing events and patient initiatives. These initiatives were helping to improve patient care and staff wellbeing and were identified on the inspection as noteworthy practices.

Patients we spoke with told us they felt safe and happy on the ward and described staff as kind and helpful. A calm, inclusive and professional atmosphere was observed throughout the inspection, with staff showing warmth and respect in their interactions with patients. Each patient had a private bedroom, supporting dignity and privacy, and the use of personalised signage helped individuals identify their rooms.  Patients enjoyed therapeutic activities such as baking and pet therapy, with regular use of outdoor garden spaces. The ward’s ‘Fresh in 15’ initiative supported daily access to fresh air and outdoor time.

Timely assessments and care were being delivered, with staff showing a strong understanding of individual patient needs. Inspectors found systems in place to support safe and effective care, including up-to-date policies, completed risk assessments, and securely maintained patient records. Safeguarding procedures were robust, and medication management was safe and well-handled. Care plans were tailored to each patient and showed clear involvement from patients, families, and the wider multidisciplinary team.

Staff reported feeling well supported and described a positive and inclusive culture on the ward. They expressed confidence in raising concerns and had a clear understanding of the appropriate procedures. Leadership from the ward manager and senior team was described as committed and passionate, with a well-defined management structure and a good team-working ethos in place. Inspectors observed ongoing quality improvement and learning initiatives, including regular staff newsletters, wellbeing events, and programmes aimed at enhancing both patient and staff experience.

While the inspection was largely positive and no immediate risks to patient safety were identified, some areas for improvement were noted. Inspectors highlighted the need to strengthen access to key specialist roles including a mental health pharmacist, dietitian, and speech and language therapist. Improvements were also recommended around staffing levels, training compliance for agency staff, and addressing potentially unsafe fixtures within communal areas.

The ward’s focus on patient initiatives is helping to improve patient care, with some examples of noteworthy practice identified during our inspection.
