How to tell us about changes to your service

Private dental practices and independent healthcare services

Private dental practices, private direct access practices and independent healthcare services that are registered with HIW must tell us in writing when certain changes take place.  These are:

  • Changes to the Statement of Purpose and Patients’ Guide/Patient Information Leaflet
  • Changes of registered person/s
  • Registered person/s being convicted of a criminal offence
  • Absence of registered person
  • Changes to the organisation
  • Appointment of liquidators
  • Death of a registered person

How to tell us

Forms are available below.  Please complete and give as much information as possible before sending them to us. Remember to include any other information you think is important.  When we have considered what you have told us we may ask for further information.

How to send the information to us

What you are telling us will often contain sensitive information.  You can either send forms by secure Royal Mail post or you can send them via Object Connect, which is a secure way of sending us information electronically.

What we will do with the information you give us

We will look at the information you send us within 10 working days of receiving it and we will keep it with your other records.  If we need any other information from you or need you to take any further action we will let you know.

The law

Regulation 7 (b), 10(2), 26, 27, 28, 29 and 30 of The Private Dentistry (Wales) Regulations 2017 state that the registered person of a private dental practice must tell us in writing when changes happen.

Regulations 8(b), 11(2), 11(3), 14, 32, 33, 34 and 35 of The Independent Health Care (Wales) Regulations 2011 state that the registered person of an independent hospital, independent clinic or independent medical agency must tell us in writing when changes happen.
