How do I register as an independent healthcare setting with HIW?Step one: Obtain DBS (formerly CRB) formAs part of your application, you will need to enclose a DBS form with the relevant documents. You will need to obtain a DBS form by contacting the DBS on 0870 90 90 811 or by emailing customerservices@dbs.gsi.gov.uk. You will have to ask for a form with the following Registered Body number – 25474000005. Guidance on completing the CRB form can be found at www.homeoffice.gov.uk/dbs. Your completed DBS form and relevant documents to be checked must be sent in with your application, with a cheque for £44.00 to cover the cost of the check. Step two: ReferencesYou will need to obtain a financial reference from a suitably qualified person (Bank Manager).
