Enhanced Criminal Records Certificate

Each completed application for dental registration must be accompanied by a CRB certificate or an application for a new CRB certificate:
  • If you provide dental services privately and are currently on a NHS dental performers list in Wales; and
  • a CRB certificate has been obtained in connection with inclusion in that dental performers list; and
  • Less than three years have elapsed since The CRB certificate was issued;and
  • There remains a minimum 6 month period from the date you make your application for registration
That certificate will be accepted for the purposes of this registration.

If the above does not apply, you will have to obtain a new certificate.  To do this you must obtain a CRB application form from HIW.

When sending an application for a Criminal Reference Bureau (CRB) certificate please make sure that the accompanying cheque is made payable to either HIW or Welsh Assembly Government. Failure to do so will delay the process as cheques will have to be returned.

This list will be updated at the beginning of every month

Please note: If your application for registration to provide private dental services is received by Healthcare Inspectorate Wales (HIW), after the transition period deadline of 30 June 2009, (for dentists who were already providing private dental services prior to 1 January 2009), you should not offer any private dental services until your application for registration has been granted.
