Report of a review in respect of Mr E and the provision of Mental Health Services, following a Homicide committed in August 2007



Healthcare Inspectorate Wales (HIW) today publishes its report following a review of a homicide carried out by a Mental Health Service user in Newport.

The report investigates the healthcare and support provided for Kevin Price (referred to in the report as Mr E) and how this had an impact on the death of James Fortey (referred to in the report as Mr V). 
 
The review has highlighted concerns in relation to the adequacy of the care and treatment provided to Kevin Price.  The findings of the report are:
  • The killing of Mr V could not have been predicted, however had Mr E been engaged more assertively in care and treatment it may have led to the likelihood of any homicide being averted. 
  • There was a lack of an assertive approach to the care, treatment and monitoring of Mr E.
  • Risk assessment arrangements were not reviewed with the frequency HIW would expect and documentation was not always completed.
  • Engagement with Mr E’s family was poor and they were not provided with appropriate support during what must have been a very difficult time for them. 
  • Compliance with medication should have been followed up more robustly.
  • Leadership and management systems operating within the CMHT were ineffective.
  • The Care Programme Approach had not been properly implemented, including inadequate attention to the assessment, identification and management of risk.
The report of the review sets out 6 recommendations.
 
Chief Executive of HIW, Dr Peter Higson, said today:
 
“There are important lessons to be learnt from this tragic case.  I urge all mental health service providers to assess themselves against the findings and recommendations we have made in our report and use them to ensure that they are better able to minimise the role of similar incidents occurring in the future.”
